This document sets out the Privacy Policy of Charlotte Parr trading as Charlotte Parr (ABN 30 018 121 279) (Charlotte Parr, “we”, “us”). We commit to protect the privacy of personal information we collect about you, including through our website, located at www.charlotteparr.com, as well as through the provision of psychology and coaching services or directly from you.

We are a psychology and coaching practice and because of the nature of the services we provide, we are bound by the Australian Privacy Principles set out in the Privacy Act 1988 (Cth) (the Act).



         Saying no to information collection

If you don’t want your personal, sensitive or private information to be collected in the way we describe in this policy, we may not be able to provide our services to you.

Where appropriate, you may request to be anonymous and interact with us using a pseudonym, unless it’s not practical for us to deal with you that way.

Please let us know as soon as possible if you have any concerns about us collecting your personal information.

         YOUR INFORMATION

1      What information we collect

The types of personal information we collect may include:

(a)           Name, gender and date of birth;

(b)           Identification information, such as your licence or photo identification;

(c)           Contact information, such as your address, phone number, email address or other contact details;

(d)           Medical information, such as medical history, medication, injuries, illnesses, and similar information to ascertain your suitability for participation in our programs;

(e)           Details of medical reports or letters from your doctor approving your participation in our services;

(f)             Medicare number and health fund details if you are eligible to claim the cost of our services back from a health fund or receive a rebate from Medicare;

(g)           Financial payment details, such as your credit or debit card number; and

(h)           Any other information disclosed by you, if relevant when providing our services directly to you, such as employment information and duties (for example, if you a rehabilitating from a workplace injury).

2      How we collect your information

We may collect information about you via our website (www.charlotteparr.com), in person, online, over the phone or through email.

If you are under the age of 18, we may request or receive your personal and sensitive information from a parent or legal guardian.

We are committed to using lawful and fair means to collect personal information and collecting it from others only when it is unreasonable or impracticable to obtain certain information from you directly.

3      Why we collect your information

 We collect, hold, use, and disclose personal information only as is reasonably necessary for our operational needs, including to:

(a)           contact and communicate with you;

(b)           provide our services to you;

(c)           assess your suitability to access other services we provide;

(d)           liaise with Medicare or your health fund, if required;

(e)           conduct activities relating to research, quality assurance and improvement processes, accreditation, audits, risk and claims management, client satisfaction surveys, and staff education and training;

(f)             handle a complaint or respond to anticipated or existing legal action;

(g)           when required for administrative and internal record keeping for a minimum of 7 years after our last contact;

(h)           for statistical purposes; and

(i)             as required by law.

4      International Disclosure

Your personal information will not be disclosed to overseas recipients, unless you give us permission to do so or we are required to do so by law. We won’t use, sell, rent, or disclose your information for any other purpose.

5    REQUESTING ACCESS

Accessing your information

You have the right to request details about the personal information we hold about you, as set out in the Act. Unless we have the right to refuse your request under the Act, we will provide access to your information if it’s reasonable to do so.

In most cases, we’ll do this for free, but if your request requires significant effort or expense (such as excessive printing), we may charge you a fee.

2      Correcting your information

If you believe that any information we hold about you is inaccurate, out of date, incomplete, irrelevant or misleading, please let us know by emailing hello@charlotteparr.com. We rely on you to keep us up-to-date when your information changes.

Unless it’s unlawful or impractical to do so, we’ll do our best to promptly correct any information you tell us is incorrect and let you know when we’ve updated it. 



3      Deleting your information

If you want us to delete your personal information or not collect information for a specific purpose, please let us know by emailing hello@charlotteparr.com. Please note that if we agree to delete your information, because of backups and records of deletions, it may be impossible to completely delete your information without retaining some residual information.

We will respond to any request to access, correct or delete information within a reasonable time.

         KEEPING YOUR INFORMATION SAFE

1      Maintaining security of your personal information

We are committed to ensuring that your personal information is secure and protected from misuse, interference, loss and authorised access. We take the following steps to protect your personal information:

(a)           Our website contains pages encrypted with SSL (Secure Sockets Layer) to ensure the safety of any data that is submitted through the use of our website;

(b)           we limit access to personal information to a “need-to-know” basis;

(c)           we protect devices we use to collect, hold, use and disclose personal information with industry-standard anti-virus software;

(d)           our devices are protected by password and are stored in secure premises;

(e)           data is securely stored on cloud servers;

(f)             our email data is encrypted;

(g)           all hard copies of personal information are kept in secure storage with access by authorised personnel only;

(h)           all conversations involving the discussion of personal information take place in private, where conversations are unable to be overheard by unauthorised personnel;

(i)             if we no longer need personal information, we take reasonable steps to delete or de-identify the information; and

(j)             we train our staff in privacy management and data breach procedures.

          

If a data breach occurs that is likely to cause harm, we will notify you as soon as possible and act in accordance with our obligations under the Act.


2      Raising concerns

If you have any concerns about the management of your personal information, you should contact Charlotte Parr at www.charlotteparr.com. We will respond within a reasonable time.

You can ask us for a copy of the Australian Privacy Principles contained in the Act, which sets out your rights about how your personal information should be handled.
